We have been asked by a number of parents to add these forms online so that the parents may download them, print them out from home, and be able to send them, bring them, or fax them to us here at the church for their kids, for the different outings and events. The links above will take you to the forms, for downloading (open OR save, your choice) The forms, filled out, can then either be faxed, attached to an email, or of course printed then brought to church.
